What you'll do

• Conduct caregiver orientation and review competency, infection-control, and care-plan knowledge
• assess clients’ health status and care needs and collect relevant histories and medication information
• develop individualized plans with clients and families
• review, update, and document care plans
• perform follow-up assessments and communicate findings
• guide and train personal care attendants.
