What you'll do

• Assess patient needs involving housing, employment, finances, legal matters, healthcare, and community resources
• coordinate aftercare, referrals, and discharge planning
• assist with FMLA, disability, court, and other paperwork
• communicate with approved contacts while maintaining confidentiality
• document case notes in the EMR
• collaborate with clinical staff and treatment team members
• participate in treatment meetings
• coordinate discharge schedules
• follow HIPAA, facility policies, and applicable regulations.
