What you'll do

• Assess patients and families to determine home care needs
• reevaluate nursing needs and update care plans
• administer prescribed medications and treatments
• provide preventive and rehabilitative nursing procedures
• counsel patients and families and provide health instructions
• identify and implement discharge planning needs
• prepare clinical notes and report changes in patient condition
• coordinate with physicians and community health contacts
• arrange necessary equipment and services
• instruct, supervise, and evaluate home health aides every two weeks.
