The Medical Benefit Review Services Associate is an onsite role based in Donner, LA, with a pay rate of $19.50 per hour. This position supports Conduent State Healthcare, LLC in delivering mission-critical services for government clients and the individuals they serve. The role exists to support Louisiana Medicaid and long-term care programs by conducting in-home assessments, coordinating services, and helping older adults and individuals with disabilities access the healthcare and community resources they need to remain safely and independently in their homes.

What you'll do

• Conduct in-home assessments for LT-PCS applicants, evaluate client needs, provide Choice Counseling for HCBS programs, educate applicants on Medicaid long-term care, develop person-centered Plans of Care, coordinate services with providers, and complete monitoring visits and documentation.
