Community Liaison

What you'll do

• Build and maintain relationships with physicians, hospital discharge planners, skilled nursing facilities, assisted living communities, social workers, and case managers
• conduct outreach visits and educational engagement
• support referral growth and census targets
• explain hospice care to patients and families and facilitate admissions
• collaborate with clinical and intake teams
• and travel regularly within the territory.
