What you'll do

• Conduct skilled patient visits
• provide daily patient interaction and support
• educate patients about conditions including CHF, COPD, and diabetes
• perform wound care and assessments
• manage and administer medications
• change dressings and bandages as prescribed
• document patient conditions and report significant changes to physicians
• and collaborate with physical, speech, and occupational therapists.
