What you'll do

• Collaborate with individuals to identify interests, strengths, and goals and develop schedules for community participation and activities
• teach tasks and goals through coaching and instruction
• follow individualized behavioral care plans
• document progress, activities, and outcomes
• provide a safe, supportive environment while respecting choice and advocating for independence and dignity.
