What you'll do

• Provide home health visits
• perform comprehensive assessments and document them in the agency portal
• obtain and document physician orders
• initiate and reevaluate care plans in consultation with the Clinical Team Manager
• coordinate care and report significant changes
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other team services
• conduct Home Health Aide supervisory visits as required.
