What you'll do
- Provide in-person and telephonic medical case management
- discuss treatment plans and alternatives with physicians
- assess, plan, implement, and evaluate patient progress
- review treatment for appropriateness, medical necessity, and cost effectiveness
- attend provider, home, hospital, and long-term facility visits or discharge planning conferences
- conduct initial home evaluations
- and coordinate durable medical equipment and nursing services.
