Registered Nurse (RN), Visiting Home Health

What you'll do

• Provide home health visits
• perform initial comprehensive assessments and document findings
• obtain and document physician orders
• initiate and reevaluate care plans with the Clinical Team Manager
• coordinate with physicians and the home care team about patient changes
• complete recertifications, resumptions, transfers, and discharges as requested
• assess need for other team services
• conduct home health aide supervisory visits.
