What you'll do

• Provide comprehensive primary care to adult patients
• evaluate, diagnose, treat, and manage acute and chronic conditions
• develop individualized treatment plans and support preventive care
• coordinate care with physicians, nurse practitioners, and clinical staff
• document in Epic
• support quality improvement and standards of care
• collaborate on clinical protocols, workflow improvements, and operational planning
• participate in practice development, quality assurance, and relevant administrative initiatives.
