Job overview
The Home Health Registered Nurse (RN) role is onsite in Kyle, TX, supporting the Buda/Kyle service area. Compensation is $80,000–$90,000 per year. Halcyon Home supports a growing geriatric home health population with a focus on compassionate care and positive patient outcomes. The nurse assesses patients, develops and documents care plans, coordinates with families and care-team members, and provides clinical guidance to hospice aides and licensed practical/vocational nurses. The role contributes to continuity, quality, and compliance of patient care.
What you'll do
- Conduct initial and ongoing comprehensive patient assessments
- apply admission and recertification criteria
- develop, implement, and document plans of care
- communicate with patients, families, physicians, and care-team members
- apply infection-control precautions
- provide clinical direction to hospice aides and LPN/LVNs
- maintain clinical and progress notes
- monitor cases for payer compliance
- educate patients and families on disease processes, self-care, end-of-life care, and nutrition
- determine service scope and frequency
- initiate preventive and rehabilitative nursing procedures.
