Hospice Admissions Registered Nurse (RN)

What you'll do

• Complete initial and ongoing comprehensive patient and family assessments
• provide nursing care using the nursing process
• write and initiate plans of care and revise them as needs change
• develop goals and palliative nursing actions with patients and families
• administer prescribed medications and treatments
• counsel and educate patients and families
• document clinical notes and changes
• communicate with physicians and community health contacts
• coordinate continuity of care
• arrange necessary equipment and services
• participate in quality improvement activities.
