What you'll do

• Assist with bathing, dressing, grooming, and toileting
• support mobility, transfers, and basic exercises according to the care plan
• help maintain a clean and safe home environment
• prepare meals and assist with feeding as needed
• take and record vital signs when required and trained
• assist with medication self-administration as directed
• observe and report changes in condition
• document care in the EMR
• participate in team meetings, training, and care coordination
• support patients and families during visits.
