Mental Health Case Manager

What you'll do

• Provide Community Psychiatric Support Treatment and Therapeutic Behavioral Services in outpatient and community settings
• establish therapeutic relationships and assess complex problems
• develop Individual Recovery Plans
• coordinate access to medical treatment, employment assistance, housing, and other services
• participate in assessments
• provide crisis assistance
• delegate recovery-plan tasks to service providers and monitor service delivery.
