What you'll do

• Assess patient functional abilities and home environment
• develop and implement individualized physical therapy treatment plans
• provide one-on-one rehabilitation care
• educate patients and caregivers
• collaborate with interdisciplinary care teams
• document evaluations and progress according to Medicare standards
• travel daily within the Lansing service area.
