What you'll do

• Support clients with grocery shopping, errands, hygiene, and medication administration
• assist with mobility, including two-person transfers and use of a Hoyer lift
• help clients work toward cooking, cleaning, and social integration goals
• transport clients to appointments and community events
• follow and document progress on Person Centered Service Plans
• communicate with supervisors and team members.
