Hospice Liaison

The Hospice Liaison is an onsite role based in Gervais, OR. Compensation is not specified for this position, which supports the sales and marketing department at Serenity Hospice, an Addus family company. This role exists to grow the company's presence by developing referral partnerships and increasing census. The Liaison is responsible for facilitating patient referrals, managing intake education, and implementing marketing programs to position the organization as a provider of choice.

What you'll do

• Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria.
• Assists in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home.
• Develops a consultative relationship in assigned accounts by making effective customer contacts and presentations.
• Develops and implements marketing and education programs to increase awareness of the company.
