What you'll do

• Assist with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene
• provide companionship and emotional support
• assist with mobility, transfers, and fall prevention
• prepare meals and assist with feeding when needed
• perform light housekeeping and laundry
• provide medication reminders as permitted by agency policy
• assist with authorized errands, shopping, and appointment transportation
• observe and report changes
• maintain documentation and communicate with the care management team.
