What you'll do

• Conduct initial client and family interviews and assess clients
• develop plans of care and set service expectations
• conduct in-home RN visits every 90 days to evaluate care
• maintain client charts in compliance with NC DHHS regulations
• support periodic surveys with the Agency Director
• assist with caregiver orientation if needed.
