What you'll do
- Provide psychosocial support to residents, families, and partners
- participate in care planning
- complete MDSs, admission assessments, PAS/RR, LOCs, and discharge documentation
- lead the Grievance Committee and follow up on concerns
- support transitions of care and Medicaid Waiver processes
- evaluate programs and activities
- provide weekly support across independent living, assisted living, and nursing levels.
