What you'll do
- Provide direct person-centered support
- follow behavior-support plans, treatment plans, crisis-prevention strategies, and trained de-escalation and restraint procedures
- assist with daily living skills, household tasks, activities, transportation, and community engagement
- document services and observations electronically
- monitor behavioral changes and communicate with supervisors
- support appointments, family contacts, and medication tasks as allowed
- follow medical, nutrition, and safety plans
- participate in team meetings and help train new staff.
