What you'll do
- Follow up with prospective clients, explain services, and arrange intakes
- conduct client intakes and obtain service agreements and plans of care
- document client information in WellSky
- introduce caregivers to cases
- conduct supervisory visits and update plans of care
- provide on-site training as needed
- coordinate with scheduling staff
- contribute to caregiver evaluations
- maintain client confidentiality and required safety and infection-control practices.
