The Home Health Nurse (RN or LPN) role provides care in a patient’s residence in Chimayo, New Mexico. The stated pay range is $35–$55 per hour, and the schedule is eight hours weekly, with Wednesday, Thursday, and Friday preferred and early-to-mid-afternoon hours preferred. Giving Home Health Care provides personalized in-home care to people affected by health conditions related to work in nuclear facilities for the Department of Energy. The nurse carries out physician-directed care and works one-on-one with patients, coordinating with family members, physicians, case managers, and other care team members. The role contributes to ongoing patient care through assessments, treatment, medication administration, monitoring, documentation, education, and collaboration with aides and attendants.

What you'll do

• Execute physician-prescribed plans of care
• document care in the system of record
• administer prescribed medication, treatment, and therapies
• assess patients and coordinate care
• monitor vital signs and intake and output
• assess skin integrity and provide wound care
• communicate changes and health information to family members, physicians, case managers, and others
• educate patients, families, and home health aides
• collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants
• provide other personal care services as needed.
