What you'll do

• Prepare patients for preventive and comprehensive assessment visits.
• Collect medical history, medication information, and vital signs.
• Support medication reconciliation.
• Perform ordered clinical procedures, screenings, and point-of-care testing.
• Perform phlebotomy and handle specimens.
• Document patient information and nursing activities in the EMR.
• Travel to patient homes and community settings.
