What you'll do

• Develop and execute community awareness strategies
• build and maintain relationships with hospitals, physicians, skilled nursing and assisted living facilities, case managers, social workers, and other referral partners
• educate partners about hospice services and eligibility
• identify referral opportunities
• follow up with referral sources
• coordinate with intake and clinical teams
• represent the hospice at events and conferences
• track outreach and referral activity
• comply with hospice regulations and organizational standards.

Why this role
