Lead Care Manager

What you'll do

• Interview members to assess medical and social care gaps
• educate members about conditions and medication
• develop individualized care plans
• respond to member inquiries
• coordinate with interdisciplinary teams and locate medical and social resources
• maintain contact through telehealth and in-person visits
• advocate with providers, arrange appointments and treatment plans, evaluate progress, and assist with barriers
• support behavioral health, substance-use, and community-resource coordination.
