Social Worker — Skilled Nursing Facility

The Social Worker role is onsite in Marlborough, MA, full-time, with a 32-hour schedule. Pay is $35–$40 per hour, depending on license. The role is part of the social services department at Marlborough Hills, an 186-bed skilled nursing facility. The Social Worker plans and directs social services to address residents’ emotional and social needs. The position coordinates care conferences, supports residents and families through placement and crises, advocates for residents’ needs, documents care, connects residents with community resources, and supports staff on resident-related matters.

What you'll do

• Plan, organize, develop, and direct social services
• coordinate and monitor resident care conferences
• work with residents, responsible parties, and families on adjustment to placement and crises
• protect residents’ health information and complete timely documentation
• advocate for residents’ personal needs
• use community resources
• support staff on resident-related issues
• communicate relevant resident information to the interdisciplinary team.

Why this role
