What you'll do

• Conduct initial assessments and develop individualized care plans
• provide home-based skilled nursing, including medication management, wound care, and symptom management
• coordinate with the care team
• educate clients and families on self-care and health maintenance
• monitor and document progress and communicate concerns to physicians
• advocate for clients
• follow company policies and regulatory requirements.
