The nurse coordinates and reassesses home health services, provides clinical care, and leads field staff to support safe, compliant care and patient outcomes. The role works with clients, families, physicians, caregivers, and agency teams to coordinate ongoing care.

What you'll do

• Conduct client assessments and direct care actions
• manage and supervise nursing and aide staff
• coordinate and document patient status and changes
• perform case management, admissions, recertifications, transitional care, resumptions of care, and clinical reviews
• provide occasional skilled nursing visits
• complete OASIS and non-OASIS assessments
• support orientations, in-service training, and supervisory reviews
• travel throughout the service area.
