The nurse coordinates and delivers professional hospice care for assigned adult, pediatric, and perinatal patients. The role develops and evaluates care plans with patients, families, and the interdisciplinary team, and coordinates services to support patient and family needs.

What you'll do

• Assess patients and develop, implement, and evaluate written plans of care with the interdisciplinary team, patient, and family
• direct and supervise care provided by LVNs and health aides
• communicate changes to the team and primary physician
• document care electronically
• provide health instruction to patients and families
• train assigned personnel
• conduct admission assessments and admissions when needed
• participate in 24/7 patient and family coverage and two monthly on-call shifts
• travel for patient visits.
