What you'll do

• Build and maintain relationships with hospitals, physician offices, case managers, discharge planners, and other referral partners
• develop and execute a territory plan to increase census and revenue
• educate referral partners on services and benefits
• identify community partnership opportunities
• support patient outcomes and efforts to reduce healthcare costs.
