What you'll do

• Provide direct care, assessments, care planning, evaluation, and education for assigned infusion patients
• communicate with pharmacists, prescribers, dietitians, care partners, and community resources
• assess home safety
• coordinate care and scheduling through approved platforms
• document encounters and submit required clinical records
• maintain knowledge of therapies and protocols
• and participate in the nurse on-call rotation, including nights and weekends.
