What you'll do

• Complete social histories and psychosocial assessments
• develop and update individualized social services care plans
• coordinate discharge planning and referrals
• provide emotional support to residents and families
• participate in care conferences
• advocate for resident rights, dignity, choice, and person-centered care
• assist with advance care planning resources and appropriate referrals
• complete documentation and assigned MDS assessments
• connect residents with behavioral health and community services
• support staff education.
