What you'll do

• Evaluate residents’ progress and service eligibility
• screen potential admissions for Medicare and Medicaid eligibility
• coordinate welcome care-plan meetings
• validate reimbursements with business and rehabilitation offices
• chair or coordinate interdisciplinary assessment and care-planning meetings
• coordinate skilled services with nursing and therapy
• monitor rehabilitation progress
• maintain accurate records and coding
• submit MDS data to CMS
• prepare quality-indicator reports
• maintain survey documentation
• follow applicable regulations and HIPAA requirements.
