Clinical Pharmacist - Abdominal Transplant

-Provide direct patient care, including medication therapy management, education and counseling for transplant recipients and living donors. -Participate in multidisciplinary inpatient rounds and contribute to clinical decision-making with the transplant team. -Monitor and adjust immunosuppression, antimicrobial prophylaxis, and other transplant-related medications in addition to chronic disease s
