Primary Care Physician

Job overview

The Primary Care Physician role is based in the Toledo service area, with a home office in Maumee, OH, and daily travel to patients’ homes and care settings. Compensation is $215,000–$238,000 per year. Optimal Care provides physician services, home health, and hospice, with a mission to serve together and deliver exceptional-quality care. The physician evaluates and treats geriatric and medically complex patients, coordinates care with families and interdisciplinary teams, and provides clinical leadership and oversight to nurse practitioners and physician assistants.

What you'll do

• Conduct comprehensive medical evaluations
• diagnose and manage acute and chronic conditions
• develop individualized treatment plans
• coordinate care with patients, families, and interdisciplinary teams
• provide medical leadership and collaborative oversight for nurse practitioners and physician assistants
• mentor clinicians
• travel daily within the service area to provide care in homes, assisted living facilities, and nursing homes.
