What you'll do

• Complete initial and ongoing nursing assessments
• conduct assigned in-home visits
• assess health status, functional needs, medications, and care needs
• complete nursing documentation
• communicate changes or concerns to the care team
• provide caregiver education and support when needed
• maintain confidentiality and follow HIPAA, company policies, and applicable Medicaid and program requirements
• travel to assigned participant homes.
