Registered Nurse Case Manager

Job overview

The Registered Nurse Case Manager role is onsite in Towson, MD, with a Monday–Friday schedule from 8:30 a.m. to 4:30 p.m. and a 30–60 minute unpaid lunch. Pay is $37 per hour. The role supports a government client and provides case management for clients awaiting appointments. The nurse assesses clients, develops person-centered care plans, monitors health and plan outcomes, and connects clients with community resources. The work supports timely, coordinated care and compliance with state, federal, and Medicaid requirements.

What you'll do

• Provide nursing assessments, person-centered care planning, and referral services
• review assigned LTSS assessment requests
• contact clients or representatives promptly
• complete assessments and care plans for people undergoing cancer treatments
• provide ongoing case management and evaluate care plans
• assess and monitor physical and mental health and identify community resource needs
• ensure compliance with state, federal, and Medicaid regulations
• provide health screening education and counseling on disease prevention, health maintenance, and mental health
• prepare patient reports.

Why this role

Six- to twelve-month contract position with possible extension or contract-to-hire consideration. Requires a Bachelor’s degree in Nursing, at least one year of case management or nursing experience, a valid Maryland RN license, and Basic Life Support certification.
