This role exists to provide comprehensive, in-home and community-based care coordination for patients with complex health needs, aiming to improve their overall well-being, increase access to preventative care, and reduce emergency room visits.

What you'll do

• Coordinate care for high-complexity patients.
• Conduct outreach via phone, home visits, and community interactions.
• Assess patient needs and set health goals.
• Ensure patients receive appropriate care and resources.
• Focus on preventative care and self-management.
• Document patient interactions in EMR.

What we're looking for

Education

High school diploma or GED required.

Experience

At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or in a similar role.
