Hospice Registered Nurse Case Manager

The Hospice Registered Nurse Case Manager role is onsite in Quitman, TX, serving patients throughout Wood County; compensation is not specified. The nurse provides care wherever hospice patients call home, supporting Concho Hearts Hospice’s mission of compassionate care. The case manager coordinates an assigned patient caseload, assesses needs, updates care plans, and manages symptoms. The role also educates and supports patients and families and coordinates with the interdisciplinary team to provide consistent hospice care.

What you'll do

• Manage and coordinate an assigned hospice caseload
• complete comprehensive nursing assessments and routine visits
• develop and update individualized care plans
• monitor symptoms and provide pain and symptom management
• educate patients and families
• communicate condition changes to physicians and the interdisciplinary team
• coordinate medications, equipment, supplies, and other needs
• participate in IDT meetings
• document care accurately and promptly.

Why this role

Requires an active Texas RN license or qualifying Nurse Licensure Compact license, current CPR certification, a valid driver’s license, reliable transportation, and auto insurance. Hospice experience is preferred, but training may be available for a qualified RN.
