What you'll do

• Provide supervision and support according to individual support plans
• promote health, safety, and independence in daily living
• assist with community integration, meal preparation, funds management, social skills, personal care, and home safety
• complete daily documentation
• help individuals navigate housing, healthcare, employment, and financial resources
• provide transportation under agency policy
• maintain required certifications and training
• participate in meetings and support-plan development.
