Case Coordinator — Home Care

The Case Coordinator — Home Care role is onsite in Gallup, NM. Pay is $18–$26 per hour. Circle of Life Home Care Anishinaabe serves seniors, children, and people with disabilities, with a mission to honor those served and help clients remain independent in their homes and communities. The coordinator supports client intake and care delivery by processing inquiries and assessments, arranging Personal Care Attendant schedules, and communicating with clients and care providers. The role also supports onboarding, supervision, training, and case-file quality to help maintain reliable services.

What you'll do

• Process Personal Care Attendant applications and assist with onboarding
• conduct required monthly home visits and client wellness visits if needed
• process new client inquiries, assessments, and requests
• maintain accurate employee records
• create PCA schedules according to care plans
• communicate service updates
• report incidents and follow through on escalated complaints and claims
• supervise PCAs
• arrange training
• conduct caseload audits
• participate in required meetings and local outreach events.
