Job overview

The Registered Nurse Care Manager – MSSP – Casual role is based in Fairfield, California, with participant assessments conducted in the home or by phone. Pay is $40 to $44 per hour. What you'll do

• Screen MSSP applicants using established criteria and forms
• conduct initial health assessments in participants’ homes or by phone
• complete the medical portion of annual reassessments in person or by phone
• complete initial MSSP contracts and related forms
• identify medical needs that may place participants at risk of injury or placement.

Why this role
