SSVF Case Manager, Amarillo

What you'll do

• Conduct intakes and housing navigation
• develop Housing Stability Plans
• provide case management and financial assistance consistent with SSVF guidance
• coordinate with the Health Care Navigator and Employment Specialist
• maintain relationships with area properties
• participate in community meetings and case conferences
• conduct Panhandle outreach
• document services in HMIS and agency records.
