What you'll do

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education
• document services to meet Medicaid reimbursement standards
• coordinate with physicians, case managers, and interdisciplinary team members
• educate patients and caregivers
• maintain timely EHR documentation
• participate in Medicaid audits, quality assurance initiatives, and case conferences
• connect patients with community resources.
