Aging in Place Coordinator

The Aging in Place Coordinator role is based at Golden Phoenix Adult Center in East Elmhurst, NY, with a Monday–Friday, 9:00 AM–5:00 PM schedule. Compensation is $31.12 per hour. Elmcor Youth & Adult Activities provides services for older adults through its center network. The coordinator oversees in-community services and staff, manages caregiver and behavioral health support programming, and coordinates resources for older adults. The role helps participants remain safe and supported in their homes through service coordination, documentation, and community connections.

What you'll do

• Oversee program supplies and coordinate benefits, entitlements, case assistance, shopping assistance, chore services, friendly visiting, and telephone reassurance
• manage evidence-based caregiver and behavioral health support programs
• supervise and train staff and volunteers
• maintain records and STARS data
• prepare reports and meet unit targets
• conduct weekly case meetings
• maintain community resource contacts
• refer older adults to activities and resources
• report unsafe conditions and resolve departmental issues.
