What you'll do

• Manage a caseload of youth with intellectual and developmental disabilities and develop person-centered service plans
• coordinate admissions, assessments, annual reviews, and discharges
• work with families, guardians, schools, and service providers
• plan activities, community outings, and skill-building opportunities
• maintain compliance with Wisconsin CLTS Waiver regulations and documentation requirements
• assist with staff training, onboarding, and program oversight
• support participant health and safety
• drive accessible vehicles for community activities as needed.
