Pharmacist Consultant

The PharMerica consultant pharmacist team provides medication management and clinical support to long-term care and senior living facilities. The role exists to optimize residents’ medication therapy, support facility compliance, and help care teams make informed treatment decisions. The consultant pharmacist reviews medication orders and resident drug regimens, communicates findings and recommendations, and applies long-term-care pharmacy knowledge.

What you'll do

• Evaluate medication orders for drug, dosage, interactions, route, and stability
• recommend therapy changes when appropriate
• review assigned residents’ drug regimens and clinical indicators at least monthly
• report irregularities and recommendations in writing to facility leaders and attending physicians
• document communications
• maintain knowledge of pharmacy laws, regulations, and long-term-care pharmaceutical care practices
• work cooperatively with the team.
