The Clinical Pharmacist — Specialty role is onsite in Cincinnati, OH, with a Monday–Friday schedule from 8:30 AM to 5:00 PM and an on-call rotation of one week per quarter. Compensation is not specified. The pharmacist supports Soleo Health’s Specialty Therapy Care Management Team, and the company provides specialty pharmacy and infusion services. The pharmacist assesses clinical information, coordinates specialty therapy care plans with prescribers, and monitors treatment efficacy and safety. The role also dispenses medications and supplies, documents patient interactions, and supervises support personnel to help deliver compliant, coordinated care.

What you'll do

• Collect and assess clinical data from patients, caregivers, and nurses
• collaborate with prescribers to implement or modify treatment plans
• provide clinical expertise to patients, prescribers, referral sources, and team members
• coordinate with pharmacy, nursing, intake, reimbursement, and sales staff
• document patient interactions and care coordination
• dispense specialty medications and supplies
• support quality improvement
• train new pharmacy staff
• supervise therapy care specialists and support personnel
• ensure compliance with laws, accreditation standards, and company policies
• obtain additional state pharmacy licenses as requested.
